One meeting registration is included for all organizations participating in the Shared Practice program. There is a $300 fee to offset the meeting cost for additional attendees. Use the form below to register for the meeting
If your organization is not a Shared Practice program participant, you will need to be approved (to see if you are eligible and there is space available) and pay the $650 registration fee. Use the form below to submit an application to attend the meeting.
